Admission Coordinator (Half-time), CRTS

Health Solutions is a premier wellness center focused on whole-person care. With almost 400 employees in Southern Colorado, you would be joining a mighty team of support and administrative staff, case managers, clinicians, physicians, nurses, and others in our efforts to improve the health and wellbeing of our community.

We’re looking for part-time (20 hours per week) Residential Admission Coordinator to join us at our Crestone Residential Treatment Services (CRTS) facility at 41 Montebello Road in Pueblo.

What You’d Be Doing

As the CRTS Residential Admission Coordinator you would provide the essential initial services that help our clients begin their journey to recovery. Acting with the kindness and compassion that is core to our culture, you would be engaging directly with clients to follow-up after initial application and help them prepare for admission to the residential treatment program, coordinating resources during and after their treatment stay, and participating as a member of our interdisciplinary team. 

Specifically, the CRTS Residential Admission Coordinator

  • Interacts with program applicants, completing interviews and determining the appropriateness for the program for each of them;
  • Coordinates and schedules appointments and admissions;
  • Assists applicants with arranging for collateral resources as needed;
  • Participates in treatment planning and review to integrate care, and effectively and efficiently utilize resources;
  • Consults with treatment providers and coordinates client care, including aftercare;
  • Provides information regarding potentially helpful community resources;
  • Monitors linkages with aftercare providers to assure effectiveness;
  • Assists clients with applications for benefits;
  • Completes post-discharge surveys routinely and manages outcomes data;
  • Works effectively as a member of the interdisciplinary team that includes the team lead and clinical staff;
  • Collaborates with the program director to identify and address problems;
  • Participates in training sessions, including those necessary to maintain safety of the milieu;
  • Maintains accurate and up-to-date records with 100 percent compliance with our standards;
  • Maintains proficiency with the electronic health record;
  • Ensures that admissions are scheduled in a manner optimizing residential census; and
  • Performs other work-related duties as assigned.

What We’re Looking For—The Must-Haves

  • Baccalaureate degree in social work, psychology, criminal justice, or a closely related field
  • Experience working with individuals with substance use and co-occurring disorders
  • Case management and community systems navigation experience
  • Exceptional documentation and computer skills
  • Self-starter possessing superior time management skills
  • Experience working successfully as part of multidisciplinary team
  • Colorado driver’s license and access to transportation during work hours

What We’d Like to See in You—The Nice-to-Haves

  • CAC II or willingness to pursue CAC credential
  • Spanish language skills
