Integrated Healthcare Case Manager

Health Solutions is a premier wellness center focused on whole-person care. With almost 400 employees in Southern Colorado, you would be joining a mighty team of support and administrative staff, case managers, clinicians, physicians, nurses, and others in our efforts to improve the health and wellbeing of our community.

We’re looking for a Case Manager to join our Care Coordination team at 1 Wellness Way in Pueblo.

What You’d Be Doing

As the Case Manager you would play a central role in helping our Health First Colorado (Colorado’s Medicaid program) members access services and take the next steps to wellness. Your work would be primarily in the office with some travel to healthcare facilities and members’ homes. You would be called upon to collaborate with other professionals within and outside our agency to assure that clients receive the very best care available.

Specifically, the Case Manager

  • Makes appropriate referrals to address mental health, physical health, and social determinants of health needs;
  • Links members and community resources effectively;
  • Conducts member home and community visits;
  • Ensures members attend scheduled appointments in the community and at Health Solutions;
  • Identifies stressors and precursors that led to the current situation or crises;
  • Identifies available coping mechanisms and natural supports;
  • Enhances problem-solving, independent living, and socialization skills among members;
  • Provides support for clients to remain in their home and community, avoiding more restrictive living arrangements;
  • Obtains information and coordinates with community providers when care services are divided;
  • Maintains competency and proficiency with the agency’s electronic health record;
  • Maintains accurate records; and
  • Performs other job-related duties assigned by the director or designee.

What We’re Looking For—The Must-Haves

  • Baccalaureate degree in social work, clinical psychology, or a closely related field
  • Computer proficiency
  • Colorado driver’s license, a good driving record, and access to transportation during work hours

What We’d Like to See in You—The Nice-to-Haves

  • Experience successfully working in multi-disciplinary teams
  • Experience working with adults with severe and persistent mental illness
  • Experience providing case management services to adults
  • Community systems navigation background
  • Spanish language skills
  • CAC training
  • Experience working with those experiencing co-occurring disorders
  • Knowledge of and experience with HUD housing rules and regulations
