RN, Integrated Healthcare Coordinator

We’re looking for a Registered Nurse to become an Integrated Healthcare Coordinator with our Care Coordination team at 1591 Taos Road (formerly 1 El Pueblo Ranch Way) in Pueblo.

What You’d Be Doing

As the RN Integrated Healthcare Coordinator you would become a central member of our Care Coordination team’s efforts to improve the medical and behavioral health of Health First Colorado (Colorado’s Medicaid program) Members. You would act as a point person for building professional relationships with medical partners and community agencies, synthesizing data to address client needs, and conducting outreach activities designed to meet the health needs of our community. 

Specifically, the RN Integrated Healthcare Coordinator

  • Functions as a liaison and advocate for Health First Colorado members between referring providers, specialty physicians, referral care coordinators or other relevant staff and community resources to include care conferences and member updates to their care team;
  • Focuses on patient self-management and increase knowledge of patient’s disease process by providing coaching, health education, home visits, follow-up and support services as well as communicate patient information and care treatment plans back to primary care medical providers; 
  • Increases patient’s ability to access timely and appropriate health care services, community resources and referrals, and guides patients through our Transitions of Care program to optimize member health outcomes;
  • Communicates with providers to get supporting documentation needed to complete service coordination plan;
  • Completes comprehensive assessments to identify needs and barriers to members ability to manage their medical conditions and treatment plans, and participates with the member, caregivers, the primary care medical provider and health care team in developing a comprehensive care plan to overcome identified needs barriers;
  • Develops strong telephonic and in-person relationships with Health First Colorado members that include assessing care coordination needs, providing health education, case coordination for Health First Colorado patients and their caregivers with complex medical and psychosocial issues, developing a joint care plan, providing member referrals to medical and non-medical resources and educating on the health care system;
  • Identifies potential program candidates by using available utilization data, hospital census data, physician referrals and referrals from other sources, and provides comprehensive nurse care coordination to include identifying and overcoming medical, psycho-social, community, cultural religious and financial barriers;
  • Coordinates care with internal and external care providers to maximize the quality of care clients receive;
  • Works effectively independently and as a team member;
  • Builds rapport and professional relationships with medical partners in the community;
  • Utilizes medical and behavioral health professional skills associated with the Patient-Centered Medical Home model;
  • Tracks and documents the progress of our members and makes course corrections when appropriate;
  • Collaborates with community agencies in service to our members and to our community;
  • Collects and analyzes data to assure that our members receive efficient and effective care to meet their needs;
  • Interacts professionally and ethically;
  • Maintains records in accordance with Health Solutions’ standards; and
  • Performs other duties as assigned.

Physical demands are light: exerting up to 20 pounds occasionally, 10 pounds frequently, or negligible amounts constantly; and requires walking, standing, and sitting to significant degrees

What We’re Looking For—The Must-Haves

  • Baccalaureate degree in nursing;
  • Current license as a registered nurse in Colorado or a state participating in the Enhance Nurse Licensure Compact
  • At least one year of case management experience or a combination of relevant experience with at least two years of work experience in community setting (e.g., home health care, medical health, behavioral health, discharge planning, department of social services)
  • Experience collaborating across disciplines and agencies
  • Colorado driver’s license and access to transportation during work hours
  • Exceptional documentation and computer skills

What We’d Like to See in You—The Nice-to-Haves

  • Previous experience with Health First Colorado
  • Knowledge of Care Transitions Coaching/Transitions of Care
  • Skilled in Motivational Interviewing
  • Excellent verbal and written communication skills and the ability to convey information clearly to both internal staff and external members/partner
  •  Intermediate level of competence on computer and internet skills required for routine data collection, data entry and reporting
  • Intermediate experience with Microsoft Outlook, Microsoft Word and Microsoft Excel Spreadsheets
  • Ability to work well in group and independent problem solving situations
  • Excellent time management skills and ability to complete tasks on time
  • Track record of working successfully as a part of a multidisciplinary team
  • Spanish language skills
