Case Manager, Adult Outpatient Services

Department: Adult Services - Chinook
Location: Pueblo, CO

Health Solutions is a premier wellness center focused on whole-person care. With over 400 employees in Southern Colorado, you would be joining a mighty team of support and administrative staff, case managers, clinicians, physicians, nurses, and others in our efforts to improve the health and wellbeing of our community.

We’re looking for a Case Manager to join our Adult Outpatient team of professionals at 1302 Chinook Lane in Pueblo.

What You’d Be Doing

As a Case Manager with Adult Outpatient Services you would play a central role in helping our clients access services and resources, and take the next steps to wellness. You would use your professional case management skills to assist clients in managing and overcoming their symptoms and difficulties to improve day-to-day functioning, support other members of the treatment team, and collaborate with professionals within and outside our agency to assure that clients receive the very best care available.

Specifically, the Case Manager

  • Makes appropriate referrals to address mental health and substance use needs;
  • Conducts client home visits;
  • Helps ensure clients attend their scheduled appointments in the community and at Health Solutions;
  • Provides behavioral health counseling aimed at identifying stressors and precursors leading to the current situation or crises;
  • Identifies coping mechanisms and natural supports that may be available;
  • Enhances client problem-solving skills, independent living skills, and socialization skills;
  • Maintains competency and proficiency with our electronic health record system;
  • Provides support to remain in home and community to avoid more restrictive living arrangement; and
  • Performs other job-related duties assigned by the director or his/her designee.

Physical requirements include the following:

  • Requires standing, sitting, driving, and walking for prolonged periods of time
  • Occasionally lifts and carries items up to 20 pounds
  • Requires working under stress in emergency situations
  • Requires normal range of hearing and eyesight to record data electronically and communicate with clients
  • Occasional exposure to communicable diseases and bodily fluids
  • Requires occasional driving in adverse weather conditions
  • Requires frequent use of hands and fingers to operate a computer keyboard and mouse

What We’re Looking For—The Must-Haves

  • Baccalaureate degree in social work, psychology, or a closely related field
  • Computer proficiency
  • Colorado driver’s license, a good driving record, and access to transportation during work hours

What We’d Like to See in You—The Nice-to-Haves

  • Experience successfully working in multi-disciplinary teams
  • Experience working with adults with severe and persistent mental illness
  • Experience providing case management services to adults
  • Community systems navigation background
  • Spanish language skills
  • CAC training
  • Experience working with those experiencing co-occurring disorders
